This notice explains how Carmel Drug Store may use and disclose your protected health information and how you can access this information.

You may revoke authorization in writing at any time. YOUR RIGHTS REGARDING HEALTH INFORMATION • Request limits on certain uses and disclosures (we may not be required to agree). • Ask for confidential communications in another way or place. • Review and request a copy of your PHI (reasonable fees may apply). • Ask for an amendment if you believe information is inaccurate. • Request a record of certain disclosures. • Receive a paper copy of this notice at any time. REVISIONS TO THIS NOTICE We may change this notice and the new version will apply to all PHI we maintain. Updated notices will be available in our pharmacy and on our website. COMPLAINTS If you believe your privacy rights have been violated, you may file a complaint with Carmel Drug Store or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
